Anti-rabies injection cost and schedule in India: free in government hospitals, Rs 310 to Rs 404 a vial in a pharmacy

Anti-rabies vaccine and rabies immunoglobulin are free in government health facilities under the National Free Drug Initiative, the Health Ministry told Parliament in August 2025. In a private pharmacy, ten vaccine brands listed on 1mg on 11 October 2026 cost Rs 310 to Rs 404 a vial. India's guidelines approve two schedules: four intradermal visits on days 0, 3, 7 and 28, or five intramuscular shots on days 0, 3, 7, 14 and 28.

Key findings

  1. Anti-rabies vaccine (ARV) and rabies immunoglobulin (RIG) are free in public health facilities under NHM's National Free Drug Initiative, the Health Ministry told the Rajya Sabha on 19 August 2025.

How much does a rabies injection cost in India?

At a government hospital, PHC or CHC, the anti-rabies injection should cost nothing. The Health Ministry told the Rajya Sabha on 19 August 2025 that ARV and RIG are free in public health facilities under the National Free Drug Initiative of NHM. Both are on the national and state essential drug lists.

In the private sector you pay for each vial. On 11 October 2026 the 1mg listing for human rabies vaccine showed ten brands priced from Rs 310 to Rs 404 a vial. Most clustered between Rs 397 and Rs 404. These are listed retail prices on one pharmacy site, and the MRP printed on a vial can differ.

A dog bite vaccine is free at government facilities. In a private pharmacy, one vial of anti-rabies vaccine was listed at Rs 310 to Rs 404 in October 2026.

Older ceiling prices exist for the vaccine. NPPA's rabies vaccine page still lists Rs 302.39 per pack (November 2013) and Rs 321.50 per pack (April 2014). NPPA's Compendium of Prices 2022 lists Rs 379.43 per pack, notified on 30 March 2022. Rabies vaccine is on the 2022 National List of Essential Medicines. We could not find a later consolidated ceiling for the vaccine in an NPPA document we could open, so we do not quote one for 2026.

What does a full course of rabies vaccine cost?

The bill depends on the schedule and on whether you need immunoglobulin. On the intramuscular Essen schedule each visit uses one full vial, so five visits mean five vials. At the October 2026 listed prices, that is Rs 1,550 to Rs 2,020 for vaccine alone.

Two published studies show what patients spent in practice. A multicentre study of 529 bite patients in six states, done in 2017 and 2018, found a median total of Rs 1,400 in government facilities. In private hospitals the median was Rs 3,685, about 2.6 times more.

A 2025 paper from a private hospital in Bhubaneswar followed 133 patients in 2024. The mean vaccine cost for those who completed the course was Rs 4,750. Immunoglobulin, given to 21 patients, averaged Rs 2,834 to Rs 2,860. Lost wages averaged Rs 3,400 to Rs 3,532 among the 24 who reported them.

Dog bite category 1, 2 and 3: which needs an injection?

Treatment depends on the category of exposure. The NCDC training module for medical officers sets out three. Category I needs no vaccine if the history is reliable. Category II needs wound washing and vaccine. Category III needs wound care, rabies immunoglobulin and vaccine.

In the six-state study, 54.4% of bite patients had category III exposures. Every one of them needs rabies immunoglobulin, which is the costliest part of treatment.

First aid matters as much as the injection. The module says to wash each wound for up to 15 minutes with soap and running water, then apply povidone iodine. It warns against chilli, turmeric, oil, lime or other home remedies on the wound.

What is the dog bite vaccine schedule in India?

India approves two post-exposure schedules, the NCDC module says. The intradermal Updated Thai Red Cross regimen gives 0.1 ml at two sites on days 0, 3, 7 and 28. The intramuscular Essen regimen gives one full vial on days 0, 3, 7, 14 and 28.

The module calls these the only regimens approved by the Drugs Controller General of India for each route. It also advises against switching route or vaccine brand during a course. Follow the dates your doctor writes on your card.

WHO's April 2018 position paper allows shorter options, including a one-week intradermal course on days 0, 3 and 7. Indian guidance has not adopted them. Your clinic will follow the Indian schedule. Pet owners can cut the risk at home too: see dog vaccination costs in India.

If the biting dog or cat stays healthy for 10 days, the module lets doctors on the Essen schedule skip day 14 and give day 28. On the intradermal schedule, the full course is given whatever happens to the animal.

Why do government hospitals give the injection in the skin?

Intradermal dosing stretches scarce vaccine. An NCDC advisory on preventing shortages, re-issued in June 2026, says the intradermal route uses 0.2 ml per visit against 1 ml for the intramuscular route. Over a full course that is 0.8 ml against 5 ml, or 84% less vaccine.

WHO's 2018 paper makes the same point in its own terms. It says intradermal regimens use at least 25% fewer vials than intramuscular ones, and up to 85% fewer as clinics see more patients. One opened vial can then serve several patients within six hours.

Uptake is uneven. In the ICMR facility survey, 224 of 372 public facilities with ARV (60.2%) used the intradermal regimen. The share was over 70% in the north, south and west, and under 40% in the north-east and east. Of 44 private facilities, 32 (72.7%) used the intramuscular Essen regimen.

How much do rabies immunoglobulin and monoclonal antibodies cost?

Immunoglobulin is the expensive part of category III treatment. NPPA's Authority approved on 7 July 2026 a revised ceiling that raised the ceiling price of anti-rabies immunoglobulin 300 IU/ml from Rs 112.19 to Rs 119.48 per ml, before GST. The revision followed a Department of Pharmaceuticals review order of 27 March 2026. That puts a 5 ml vial of 1,500 IU at up to Rs 597.40 before tax.

The amount needed depends on body weight, so a doctor calculates it for each patient. The NCDC module gives the maximum as 40 IU per kg for equine RIG and 20 IU per kg for human RIG. Human RIG is far dearer per vial.

Monoclonal antibodies are a newer option. WHO accepts a cocktail of at least two antibodies as an alternative to RIG, and the NCDC module notes that one product has been licensed by DCGI. The module's expert group wanted more Indian data before adding them to national guidelines.

Is the anti-rabies injection free in government hospitals?

Yes, by policy. The NRCP module lists rabies vaccine as mandatory at medical colleges, district hospitals, CHCs, PHCs and Health and Wellness Centres. Immunoglobulin is mandatory down to CHC level. Karnataka's October 2023 circular made ARV and RIG free for every animal bite victim, with or without an APL or BPL card.

Stock is the weak link, as our rabies vaccine access report shows. An ICMR survey of 467 public facilities, done between March 2022 and August 2023, found ARV in 372 (79.7%) and RIG in only 95 (20.3%). A quarter of facilities with ARV (96, or 25.8%) reported at least one complete stock-out in the past year.

The Supreme Court has made stock a legal duty. Its order of 7 November 2025 in the suo motu stray dog case covers every hospital. All government and private hospitals shall keep a mandatory stock of anti-rabies vaccine and immunoglobulin at all times. The Health Ministry circulated the direction in a December 2025 office memorandum.

Many bites come from community dogs; see street dog population data. If a facility has run out, ask to be referred to the nearest one that has stock. NCDC's June 2026 advisory tells states to report vaccine shortages to NPPA, the Department of Pharmaceuticals or the Health Ministry.

The list has not grown since March 2026. Health Minister J P Nadda told the Lok Sabha on 24 July 2026 that 284 model anti-rabies clinics had been set up. He added that data on how many people receive the vaccine is not kept centrally.

Is there enough rabies vaccine, and do people finish the course?

Supply is not the main gap, according to the government. Deaths are tracked in our dog attack deaths report. Health Minister J P Nadda told the Rajya Sabha on 24 March 2026 that India can make 8.17 crore ARV vials and 1.77 crore RIG vials a year. A Lok Sabha reply reported by The Week said states procured 1.33 crore vials in 2024 and used 1.26 crore.

Completion is the bigger problem. In the ICMR community survey of 2022 and 2023, 20.5% of people bitten by a dog got no ARV at all. Of 1,253 who got one dose, 615 (49.1%) did not finish the course.

Starting the injection is not enough. The NCDC module says the first three doses, on days 0, 3 and 7, should be completed within 7 to 10 days.

The module says a one- or two-day delay does not mean restarting the course. Later doses keep their original dates counted from day 0. If you miss doses in the first week, go back to the clinic and follow the doctor's advice.

Do I need the vaccine again if a dog bites me after a full course?

The NCDC module says no vaccine is needed if you finished a full course less than three months ago; only wash the wound. After three months, you need two doses on days 0 and 3, without immunoglobulin. You must be able to show proof of the earlier course.

What should I do right after a dog bite?

Wash the wound for up to 15 minutes with soap and running water, then apply povidone iodine. Do not put chilli, turmeric, oil or lime on it. Go to the nearest anti-rabies clinic the same day and follow the doctor's advice.

Vaccine and immunoglobulin prices are listed prices on 1mg product and generic pages, checked on 11 October 2026. They are retail listings, not an audited MRP survey, and several products were marked sold out. The immunoglobulin ceiling is from the minutes of NPPA's Authority meeting of 7 July 2026. The 2022 vaccine figure is from NPPA's Compendium of Prices 2022.

Course costs are our calculation: number of vials multiplied by the lowest and highest listed vial price. The intradermal saving uses the NCDC figures of 0.2 ml and 1 ml per visit. The model clinic count comes from the NRCP list updated on 26 March 2026, which we counted by state in Python.

Availability, completion and patient cost figures come from peer-reviewed studies by ICMR and Indian medical colleges. We name each study's period and sample size where we use it.

Limitations. Retail prices change by batch and seller, and 1mg pages may lag behind the printed MRP on a vial. We could not find a current NPPA ceiling price for the rabies vaccine itself, so we show only the 2013, 2014 and 2022 figures for context. The patient cost studies are small and old; the 2025 study covers one private hospital, and its subgroup rows do not add up to its stated totals. The clinic list counts only model anti-rabies clinics, not every facility that gives the vaccine. Nothing here replaces a doctor's assessment: follow your doctor on the category, schedule and immunoglobulin dose.
